Cindy Osborne has an extensive background in multiple areas of home health. Twenty-four years ago, she began her career as a field clinician for a major urban hospital based home health company. Moving quickly into clinical management, she developed her management skills supervising a large multidiscipline team of clinicians for the same agency. After several years, the opportunity arose to move into a quality improvement role. This was a position that allowed Cindy to expand her skills into Coding and OASIS certifications, and she educated all staff members on OASIS and the importance of accuracy in all areas of documentation. Cindy joined 5 Star Consultants in 2015 as a Coding Manager. She enjoys working with clients and the 5 Star Coding team, helping to ensure all are successful in their careers and to maintain a level of excellence in the industry.
In the past, Cindy also has worked with the Missouri Alliance for Home Care on the development of the MAHC10 Fall Risk Assessment tool and was a member of the Missouri Alliance for Home Care Conference Planning Committee.
Why We Do What We Do
With many years in healthcare management, Sharon Litwin and Linda Hoge endured countless occasions when outside consultants would evaluate operations and make recommendations based on boilerplate templates. Often these consultants had no experience in the home health or hospice industry, but relied on standardized formulas.
When we formed 5 Star Consultants in 2003, we were determined to approach each client as unique. We take into consideration that everyone has a reason for doing things a certain way, and make our recommendations for improvement based on these reasons. While we do have tools that we have developed to assist our clients, we customize according to each client’s needs.
While using a “cookie-cutter” approach might save us time and effort, we feel that individual clients’ needs are more important.
And that’s why we do what we do.
Conferences & Other Events
“5 Star’s team not only has a quick turnaround time frame, but their review notes are professional, easy for clinicians to follow and their explanations are always accompanied with recommendations and education for the staff. There is excellent communication between the coders and the clinicians. I would highly recommend Sharon and her team at 5 Star Consultants.”
Needless to say, I and all of my staff sincerely appreciate all that you ladies do for us. I can pay any company the same money to code for me, but the personal touch, the support and expertise shown to me here is priceless and makes all the difference!
Ladies, I salute you!
“Thanks for all of your support this year. We are going into our CHAP survey pretty confident thanks to 5 Star’s help.”
"Your insight into state survey regulations and the Mock Surveys were very much appreciated by all of the staff. Your knowledge and willingness to teach our staff has truly transformed our home health agency. I owe this transformation all to 5 Star Consultants."

"Our agency first began working with Five Star Consultants in 2005. Sharon provided wonderful assistance and support during the Hospice Condition of Participation changes in 2008. Without her assistance in interpreting these changes a difficult time would have been so much more difficult. She has provided invaluable education to our staff regarding case management and many other topics."
“5 Star Consultants has turned around our home health agency. In the past six months, our patient census has increased 222%!”
"5 Star Consultants has consistently brought us results. From re-engineering our agency for continued success under PPS, assuring our compliance with Medicare certification requirements, to staff education on OASIS and case management as well as serving as an outsourced team for coding and quality improvement audits, 5 Star Consultants has helped us in so many ways. Sharon Litwin and her team are reliable, trustworthy and savvy experts that have made our home care agency one of the strongest service lines for our hospital.”
"Sharon has provided us with great insight into what a QAPI program should look like and we continue to use the forms, etc she provided us. Our agency purchased Five Star’s policy manuals for both home health and hospice, they are a wonderful resource and again makes things much easier for our agency. Sharon has made visits to our agency several times, we always learn so much and enjoy our time with her."
